​​The Goal of the Appleton Clinic is to provide the most personalized and attentive Mental Health/Psychiatric medication management care possible. The Appleton Clinic offers evidence-based, individually tailored treatments for mental health conditions, including depression, bipolar disorder, anxiety disorders, ADHD, PTSD/trauma, and substance addiction.

Through our annual membership model, clients receive a tailored and comprehensive treatment plan alongside executive services, such as evening and weekend consultation hours, flexible options for in-person, virtual, and at-home appointments, prompt and efficient medication refills, and guaranteed psychiatric medication management or psychotherapy appointment availability within 2-5 business days. (We always strive to schedule clients with urgent needs for a same-day or next-day appointment when possible.) Our approach is mindfulness-based and collaborative. Dr. Cheong Chan, MD, is a board-certified psychiatrist licensed to practice in the state of Wisconsin. He completed his medical education and residency training at Rush University Medical Center in Chicago, where he also received specialized instruction in psychodynamic psychotherapy through the esteemed Chicago Psychoanalytic Institute. During his residency, Dr. Chan was appointed Chief Resident of Inpatient Psychiatry. Prior to matriculating in medical school, he conducted research at Brigham and Women’s Hospital in Boston, the primary teaching hospital of Harvard Medical School. Dr. Chan has contributed to the medical literature on a range of topics.
Dr. Chan established The Appleton Clinic in response to his concerns regarding the prevalent uniform approach to mental health care and the shortage of resources for patients who need timely, personalized, and attentive treatment.He advocates for a collaborative model of care, emphasizing the development of personalized treatment plans that integrate pharmacological, psychological, social, and spiritual dimensions.
Dr. Chan provides private sessions for medication management and psychotherapy, as well as one-time consultation services.

Are you a medical doctor and what is the difference between a Psychiatrist (a medical doctor) and a Psychologist?

​I am a board-certified physician specializing in Psychiatry, with a clinical focus on Mental Health and Addiction Medicine. As a Psychiatrist, I am uniquely qualified to provide comprehensive care by both prescribing medications and delivering psychotherapy to address a broad spectrum of mental health conditions. In my practice, I frequently collaborate with Psychologists—highly trained professionals who specialize in delivering individualized care through psychotherapy (commonly referred to as talk therapy). While Psychologists are integral to the mental health care system, it is important to note that they generally do not hold prescribing privileges.

​​​Do you offer both in-person and Video appointments?
​
To best serve our clients, we offer flexible scheduling options, including in-person, video and at-Home appointments, with availability during weekdays, weekday evenings, and weekends. We utilize a fully integrated, secure electronic patient portal that allows clients to coordinate care, receive appointment reminders and messages, and communicate directly with their clinician. All video appointments are fully HIPAA-compliant, ensuring the highest standards of privacy and confidentiality.

Do you accept insurance?

Insurance & Payment Information

Dr. Chan is out-of-network with most insurance plans, with the exception of Medicare. Depending on a patient’s out-of-network benefits (excluding federal insurance programs), reimbursement may range from 0% to 80% after the out-of-network deductible has been met. A detailed reimbursement document (Superbill) is provided on the same day as your appointment, making it easy to submit to your insurance provider.
To determine your coverage, we recommend contacting your insurance company directly to inquire about your out-of-network mental health benefits, including coverage for telemedicine and video visits. In many cases, patients may also use Flexible Spending Accounts (FSAs) or Health Savings Accounts (HSAs) to pay for mental health services.

Wonderful! To schedule your 50–60 minute initial intake appointment, please contact us at office@theappletonclinic.com or call 920-315-8443.

Prior to your appointment, you will be asked to complete a comprehensive intake questionnaire (approximately 30 minutes to complete). Please also have any relevant prior medical or mental health records available. To confirm your appointment, all intake forms—including a payment authorization form—must be submitted at least three business days before your scheduled session.

Initial Intake Appointment Fee: $575
This fee is applied toward your first year of membership and covers its entire cost. -
Follow-Up Appointments (25–30 minutes): $250 each
These will be charged to the card on file, with frequency tailored to the individual’s treatment plan. -
A Superbill will be provided on the day of each visit for insurance reimbursement.
Annual Membership & Concierge Care
The annual membership renewal fee is $575, which is generally not reimbursable by insurance, FSAs, or HSAs. However, this membership grants you:

The Appleton Clinic operates on a concierge membership model, serving a limited number of clients to ensure an exceptional level of individualized attention and care.

What should I expect at the initial intake appointment?
​
The initial session is a comprehensive evaluation lasting approximately 55 to 75 minutes. During this appointment, I will meet with the patient—and, when appropriate, their significant others—to gather essential information. This includes the reason for the visit, relevant history, current symptoms or concerns, and the patient’s goals for care.
At the conclusion of the session, I will provide a summary of my initial clinical impressions, outline recommended next steps, and discuss potential treatment options, which may include psychotherapy and/or psychotropic medication if appropriate.All information from the appointment will be carefully reviewed and, with consent, shared with the patient’s primary care physician or other relevant providers to ensure coordinated care. A follow-up appointment is typically scheduled within 1 to 2 weeks of the initial evaluation, depending on clinical need.

What Happens Once my Treatment Goals are Achieved?

As part of your continued care, I will oversee the ongoing management of your mental health treatment, with a focus on monitoring and adjusting psychiatric medications as needed. Once your condition has stabilized, a minimum of two follow-up visits per year is required to maintain membership and ensure continuity of care. Should you choose to transition your mental health care back to your primary care physician or another provider, we will gladly assist in coordinating a smooth and timely transfer of care.
